What to expect from PTI
Physicians can expect prompt feedback about the care your patient gets from PTI including:
- Acknowledgement we’ve received the referral
- Areas of concern
Patients can expect to be contacted within 48 hours of referral, once we receive their intake packet by fax.
What we need from you, the referring professional
Please have your patient download and complete the intake packet and either fax or bring it along with them on their first visit. Please advise your patient if we receive the packet by fax, a PTI representative will contact them to discuss the intake process.
In order for us to provide ongoing updates please provide the following information by fax:
- Email address or fax # where we can send updates
- Point-of-contact to receive feedback
- Patient Name, Phone Number and Referring Professional’s concerns
